insurance providers

The jobs I've been doing over the last year are through other people but as I'm starting on my own I need to get my own insurance sorted out.

Any reccomendations on who to use? And this dreaded thing rears it's head again but do insurance providers require proof of quals/CSCS card. When I was doing my bike co a few years ago quals didn't come into it but times may have changed.
