Brush it down, give it a good soak and buy a bag of easifill 60 and give it a couple of coats then sand it mate. Assuming the rads going back that is.

There has been moisture in the wall - damp. Depending on the reason it may or may not dry out. Check the pipes and see if they have leaked and caused it.
